No matter how many years have passed, you simply have to send an email to your former professor, whose recommendation you want. In your email, introduce yourself, state your major and your graduation year, remind the professor the kind of relationship you had with them (maybe they taught you a course or they were your mentor), and then politely ask for a recommendation.
